Charitable Donation Request Form
The Drake Athletics Department will fulfill requests for charitable donation when possible
Submission of form does not guarantee the request will be fulfilled
Please allow at least two weeks to process your request

NCAA COMPLIANCE RULES
It is impermissible, per NCAA rules, for the Drake University Athletics Department to provide donations to high schools or any organization, which benefits students in grades 9-12
TICKETS
Charitable donation form must be completed and approved by Director of Compliance
Tickets will be provided for Football, Men's and Women's Soccer, Men's & Women's Basketball, Volleyball, Softball and Drake Relays at the discretion of the Drake Ticket Office
Ticket donations are subject to availability and a pre-determined number of tickets designated for donation requests
Tickets will be provided in the form of gift certificates or vouchers
MEMORABILIA
Charitable donation form must be completed and approved by Director of Compliance
Items to be autographed will not be provided by Drake University
Approved autograph request items must be brought or mailed to Drake Athletics Department accompanied by a self-addressed and prepaid return envelope
STUDENT ATHLETE AUTOGRAPHS
Requests for student-athlete autographs will not be honored. Due to time commitment as well as NCAA rules regarding this topic, we cannot process requests for student-athlete autographs

All charitable donation request forms must be returned by:
